The Forms and Publications office is currently accepting orders for delivery in September.  An LSC may place one order anytime during the order cycle month.  Orders to be included in the September delivery must be received by the end of the business day on Friday, September 5th.  Orders are processed routinely and are usually shipped the last week of the month.  If your order is received after the deadline, it will be placed in the pending file for processing and shipping on the following order month.

Forms and publications available in print can be found on this website.  If the item is not listed, it is not available as a stock item.  You may also download items from the Smiley Forms section on the intranet.

To order non-stock items, please submit a print request form thru the HHSC website and include your program's Div/Pac in the charge to box.

How to Order

Everyone is welcome to browse the Forms and Publications catalog, however; only an assigned Local Supply Coordinator (LSC) for DFPS may place an order on this website.  If you would like to order items and don't know your assigned LSC, please check the LSC List for contact and mail code.

Login to begin shopping.  Only one order may be placed per ordering cycle.  Prior to placing an order, notify the Forms and Publications office of any changes to your profile or if you move to another location.
